| What are Village Family Practice's OFFICE HOURS ? | Village Family Practice's regular office hours are Monday - Friday, 7:30am - 5:00pm. Patients are seen on an appointment basis. For your convenience, we also offer extended evening hours for urgent care visits on Monday, Tuesday, Wednesday and Thursday evenings from 5:30pm - 7:00pm, in addition to Saturday mornings from 8:30am - 11:30am. Our appointment desk will gladly assist you between 7:30am and 5:00pm Monday through Friday at 713-461-2915 to schedule an appointment time. You can also request appointments through our on line request form. We encourage you to schedule routine appointments with your established physician. Should an occasion arise when your doctor is unavailable, please feel confident in scheduling an appointment with any of our physicians. Appointments are usually available on a same-day or next-day basis. Patients with acute illness will receive priority treatment. Patients who walk in without an appointment will be seen as soon as a physician is available. |
| What if I have an AFTER-HOURS EMERGENCY ? | A physician is available for urgent medical problems at all times when the office is closed. The on-call doctor can be reached through our answering service by calling our regular office number at 713-461-2915 and following the directions. Please use discretion when calling after-hours as this is for urgent medical problems only. In cases of medical emergencies, go directly to the appropriate emergency room. |
| What INSURANCE PLANS does Village Family Practice accept ? |
Village Family Practice participates with most major insurance companies. We are providers for many HMO, PPO and indemnity insurance plans. If you have medical insurance, please provide that information to our staff when you check in for your appointment. We will be happy to file claims with your insurance company on your behalf. Please be advised that different insurance companies may cover different services, and some service you assume to be covered may not actually be covered. | How can I REFILL MY PRESCRIPTIONS ? | It is our policy to fill routine prescriptions during scheduled office visits. It is our intention to provide you with ample medication until you need to return for your next visit. Please contact your pharmacist if you have additional refills remaining on your prescriptions. Refill requests by telephone or fax to Village Family Practice will not be accepted. Please check with your pharmacy regarding the status of your refills. When you are given a new prescription be sure to FILL it. DO NOT give it to the pharmacy to put it "on hold." Narcotics, pain medications and sleeping aids cannot |
| How can I get a REFERRAL TO A SPECIALIST ? | Some insurance plans require that specialist referrals be provided by a primary care physician. Our policy is that an office visit is required for all new referrals to a physician or for medical conditions for which you have not been seen in our office.. This policy is to familiarize your primary care physician with your medical condition and to improve communication between your PCP and specialist. Our referral coordinators will be glad to answer your questions by phone or e-mail. For referrals from Doctors Broussard, Domask and Feldman, contact: extension 363. For referrals from Doctors Erie, Fields, Groff, Lemming, Noel and Su, contact: extension 314 or 328.
